All patients have the right to expect appropriate
services regardless of sex, race, economic status, educational level or
religious affiliation.
All patients have the right to expect considerate and
respectful services.
All patients have the right to know who is responsible
for providing services and their professional qualifications.
All patients have the right to expect that all services
provided are held in the strictest confidence. No information will be
released to other sources without the written approval of the patient or
their legal guardian.
All patients have the right to be fully informed of
their illness and the proposed treatment.
All patients have the right to have all of their
illness and treatment answered fully and to their personal satisfaction.
All patients have the right to refuse treatment
regardless of the advice of the attending provider.
All patients have the right to know the cost of
services including contractual service costs prior to receiving care. A
copy of the fee schedule is available upon request for review.
All patients have the right to appeal to the Center’s
Executive Director any questions concerning services obtained at the
Center.
Patient Responsibilities
Patients are responsible for their own behavior and are
expected to treat staff and fellow patients with respect and courtesy.
Patents are expected to make and keep appointments for
services. As a courtesy to staff and to allow for substitutions, a 24-
hour notice is requested for cancelled appointments.
Patients with third-party payment insurance must bring
their cards and/or insurance forms with them at the time of their
scheduled appointments to allow for timely billing of services.
Patients are expected to pay fees at the time of
service. Discounts are available for eligible patients.
