CLINIC SERVICES INFORMATION

San Diego American Indian Health Center strives to promote excellence in healthcare with respect for customs and traditions.

ELIGIBILITY

The San Diego American Indian Health Center provides medical, dental and behavioral health services to any person(s), regardless of age, ethnos or sexual orientation.

Verified American Indians may receive the following services at no cost:

• Medical Office visits (limited labs and medications are available at the clinic)
• Behavioral Health visits
• Routine Dental care, exams, x-rays, cleaning, fillings (does not include services requiring labs)

American Indians are responsible to pay for labs, x-rays, medications or referrals which are not covered by insurance or a special program. Medical labs and x-rays not covered by insurance or a program are charged to the American Indian patient based on SDAIHC cost; dental labs not covered by insurance are charged to the American Indian patient based upon SDAIHC cost.

American Indian patients will be screened for special program eligibility, such as Medi-CAL. You may ask why we screen for local, state or federal assistance. Indian Health Services funds will cover only a limited a limited scope of services (such as primary medical care). If you qualify for local, state or federal insurance programs you may be eligible to receive additional benefits outside of SDAIHC. When SDAIHC can bill special programs or insurance for your services, our revenue is increased which in turn makes provisions for more services. We have a benefits coordinator who can assist you with the application process.

PAYMENT

Payment policies are set forth by the San Diego American Indian Health Center Board of Directors and can not be modified by clinic staff.

Payment is required at the time of service. S.D.A.I.H.C. accepts payment by check or cash only; no credit or debit cards. SDAIHC does not have payment plans.

MEDICAL OFFICE VISIT: Medical office visit charges are for the time with the provider only and do not include labs or medications. Patient Services will provide an estimate of your office visit charge, but actual visit charges are not available until after your appointment with the provider.

Private Insurance

• SDAIHC is contracted with some HMO insurance plans and accepts many PPO plans. You may contact Patient Services for information regarding the plans the center accepts.
• Please note that SDAIHC is not responsible for the cost of services that are not covered by insurance.
• Co-payments are due at the time of service. Patients are responsible deductibles, non-covered services and any balance due after the insurance is billed; a monthly statement will be sent to the Patient.
• Patients must bring their insurance card with them at the time of the visit.
• SDAIHC accepts payment by Cash or Check only: Debit or Credit Cards are not accepted. Prices are subject to change without notice.
• Patients who do not have insurance may apply for a discount.

Medi-Cal and/or Medicare

• SDAIHC accepts Medi-CAL
• Patients must bring their Medi-Cal card with them at the time of visit.
• Co-payments and share of cost are due at the time of visit.
• The SDAIHC is not responsible for the cost of labs, x-rays or pharmaceuticals that are not covered by Medi-Cal

Self-Pay

• Patients who do not have private or third-party insurance coverage may be eligible for services at a discounted price.
• Patients may apply for a discount by completing an “Application for Discount” form.
• All uninsured patients will be screened for special program eligibility, such as Medi-Cal.
• Labs and x-rays are charged to the patient based on SDAIHC cost.
• Patients are responsible for the cost of medication not covered by special programs.
• Dental Lab Fees: Patient is responsible for all lab costs; SDAIHC does not bill for any labs. Please note, should you discontinue treatment you are still liable for lab fees already incurred by SDAIHC on your behalf.

Billing Inquiries

• A receipt will be provided for any payment received while visiting the clinic. Patient is responsible to provide copies of receipts for any billing inquiries.
• All billing inquiries should be addressed to the SDAIHC Fiscal Department, 619-234-2158 ext. 118. Business hours are Monday- Friday 8:00 a.m. to 5:00 p.m.
